BERKS JAZZ FEST MARKETPLACE ONLINE APPLICATION FORM

Before proceeding, please have all your information ready including the required document uploads:

•  3–5 High-Resolution Photos: Showing your individual products/crafts emphasizing the jazz theme.

• 1 Booth Photo: A photo showing a past layout or mockup of your structural setup.

• Certificate of Insurance (COI): All selected vendors must submit a valid COI before setup showing general liability coverage of at least $1,000,000 per occurrence and $2,000,000 aggregate. Berks Jazz Fest, LLC and the DoubleTree by Hilton Reading must be named as additional insured.
